- Use your Mastercard or Visa by completing the credit card information on the form.
- Complete the "bill to" section of the form if your employer is paying for you to take the class.

In order to read and print the Vernon College forms and publications that have been made available in the ADOBE Portable Document Format (PDF), you need the ADOBE Acrobat Reader installed on your computer. If you do not have this, you can download it free by clicking on the icon below then clicking on the link to the free Acrobat Reader. You will then be guided through the download process.
Student Accident Reports Form
Acrobat Reader can be used without restrictions or copyright violations.
For your convenience, you may use your computer as a typewriter and complete any form marked with an * before you print it. Simply move your cursor to the first field on the form, left click, and begin typing. The tab key will allow you to navigate through the form. After completing the form, you MUST print it, sign and date it if required, and mail or bring it to the appropriate office.
IMPORTANT: After you print your completed form, you MUST take an additional step to protect your privacy. It's simple. Go to the very bottom of the form and click the Reset Form button. The data you have typed (name, social security number, etc.) is stored in a cache for use in other forms. Clicking the Reset Form button clears all of your entries. Until you clear them, they remain in a cache and may be viewed the next time the form is opened.
Clicking the Reset Form button is especially important if you are using a public access computer, such as in a library or computer lab, where other people might view the form and have access to your personal information.
The reason it is necessary to clear the form is that Acrobat has a feature allowing data to be entered once and used to fill multiple forms. The necessity to reset the form is not due to a flaw in the Vernon College forms.
